Because all fire art is separately reviewed, we ask that this form is filled out in addition to your Art registration.
Either one of those two will serve as your Art Registration and this form will be given to the Fire Safety Committee to review 🙂 Thank you for filling both out! With any of these forms you should get an email confirmation of your form submission.
You will need to provide a list of the safety equipment you plan to bring and a detailed safety plan. Your art, your responsibility!
